Q: How do I schedule an appointment for myself or a loved one?
A: Once you have obtained a referral from your doctor or therapist, you can call our office at (423) 648-4898 to schedule an appointment.
Q: What should I bring to my appointment?
A: For your first appointment please bring the following:
- Photo ID
- All insurance cards
- Prescription (Rx) from your doctor, or referral from your therapist
- Copies of any imaging you may have related to your condition
- Any items related to your care such as shoes, orthosis, or prosthesis
- Completed new patient paperwork packet you received in the mail
Q: How long will my appointment last?
Q: Is there a co-pay for my appointment?
A: You are not responsible for a co-pay for appointments at our office.
Q: How long will it take to receive my orthosis?
A: After evaluations we will submit for prior authorization with your insurance. Once approved your device will be fabricated and our office will schedule your appointment for fitting.
Q: What do I wear to my appointment?
A: This depends on your care needs:
- Scoliosis Bracing: Girls, please bring or wear a one piece swim suit. Boys, please wear loose pants and a tight fitting compression t-shirt.
- Foot Orthotics: Please bring all shoes you want the orthoses to fit into. Bring or wear shorts.
- Cranial Helmets: Please bring a change of clothes to your child's casting appointment.
- All Lower Extremity Orthoses (AFOs, KAFOs, Knee Orthoses) and Prostheses: Please bring or wear shorts.
- All Upper Extremity Orthoses and Prostheses: Please bring or wear a short sleeve shirt.
